What Is Iboga & Ibogaine?
Iboga is a sacred plant medicine from the rainforests of Central West Africa, used for millennia by the Bwiti people of Gabon as a sacrament of initiation and healing. Its primary psychoactive alkaloid, Ibogaine, has been studied for decades as a profound interrupter of addiction, a deep trauma processor, and a catalyser of neurological renewal. It is unlike any other substance on Earth.

The Titration Protocol
Unlike many providers, we titrate the medicine slowly over 12 hours, beginning with small test doses and building incrementally. This approach dramatically reduces cardiac risk, allows real-time monitoring, and enables us to calibrate the experience to each individual's physiology and intention.

Iboga vs. Ibogaine HCl
We work with both full-spectrum Iboga root bark / Total Alkaloid extract and purified Ibogaine HCl depending on the individual's needs. Full-spectrum iboga carries all plant alkaloids for a more traditional, holistic experience. Ibogaine HCl offers more precise dosing for medical detox contexts.

Medical Safety Framework
Every participant undergoes full medical screening: 12-lead EKG, comprehensive blood panel, liver function, electrolytes (Mg, K, Ca), and full medication review. Heart rate and well-being are monitored throughout. Our titration-based protocol is designed to be inherently safe, and clear emergency protocols are in place. Safety is our first medicine.

MEDICAL SAFETY
Our Safety Protocol
Ibogaine is a powerful medicine that demands complete medical rigor. We take every precaution to create the safest possible environment. Safety is not an afterthought — it is the foundation everything else is built upon.
Pre-Treatment Screening (Required)
- 12-Lead EKG — Screens for QT interval prolongation, arrhythmias, and any pre-existing cardiac abnormality. A QTc above 450ms is a contraindication.
- Full Blood Panel & Liver Function — AST, ALT, GGT, ALP, bilirubin, albumin. Ibogaine is fully liver-metabolized; impaired function is a contraindication.
- Electrolyte Panel — Magnesium, potassium, calcium, sodium. Low Mg and K independently prolong QT. These must be corrected before treatment.
- Complete Blood Count (CBC) — Screens for anemia, infection, platelet abnormalities, and general hematological health.
- Full Medication & Substance Review — Every medication, supplement, and substance is reviewed for interactions. Many drugs require tapering and washout periods before treatment can safely proceed.
- Thyroid Panel (TSH, T3, T4) — Thyroid dysfunction affects cardiac rhythm and is screened for in all participants.
During Treatment
- 12-Hour Slow Titration — We begin with a small test dose and build the medicine slowly over 12 hours. This is the cornerstone of our safety approach, allowing real-time cardiac monitoring and dose calibration.
- Heart Rate Monitoring — Heart rate is observed throughout the experience. Our slow 12-hour titration approach allows us to stay closely attuned to each participant's physiological state, making gentle adjustments as needed and ensuring the body is never overwhelmed by the medicine.
- Nervous System Relaxation Protocols — Breathwork, grounding techniques, somatic support, and calming practices are available throughout the session to support the nervous system if needed, ensuring participants feel held and safe at every stage of the experience.
- Electrolyte Optimization — IV or oral magnesium and electrolyte support is standard in the 24–48 hours surrounding treatment.
- Full Sober Supervision — At least one team member remains awake, present, and medically alert throughout the entire experience. No participant is ever left alone.
- Emergency Protocols in Place — While our titration-based approach is designed to be inherently safe, we maintain clear emergency protocols and have established relationships with local medical services should they ever be needed.
PREPARATION PROTOCOL
The 2-Week Supplement Protocol
We provide every participant with a full, personalized supplement protocol beginning 2 weeks before treatment. After addiction and detox, the body is depleted. Rebuilding before treatment dramatically improves safety and the depth of the experience.

Cardiac & Electrolytes
Magnesium glycinate, magnesium L-threonate, potassium (food + supplement), calcium, and daily electrolyte drinks. Low magnesium and potassium independently prolong QT — correcting these is the single most important safety preparation.

Liver Support
Milk thistle (silymarin), NAC (N-Acetyl Cysteine), Alpha Lipoic Acid, choline. Ibogaine is metabolized entirely by the liver — a healthy, supported liver is critical for safe metabolism and reduces toxicity risk.

Nervous System Repair
Thiamine B1 (critical for alcohol history), full B-complex, methylated B12, omega-3 fish oil (2–4g EPA+DHA), vitamin D3+K2, zinc, methylfolate. Addiction depletes these systematically — rebuilding creates the neurological foundation for the ibogaine experience.

Adrenal & Stress Recovery
Ashwagandha (KSM-66), L-theanine, glycine before bed, phosphatidylserine. Detox stresses the adrenal system severely. Stabilizing the HPA axis improves resilience and prepares the body to face the intensity of ibogaine.

Gut Health & Immunity
High-potency probiotic (50B+ CFU), L-glutamine, digestive enzymes, vitamin C, collagen peptides. 90% of serotonin is produced in the gut — healing the gut-brain axis directly supports ibogaine's serotonergic therapeutic mechanism.

Neurotransmitter Precursors
L-tyrosine (dopamine), DLPA (endorphins), L-tryptophan (serotonin — stopped 2 weeks before ibogaine). Addiction strips the brain of its neurotransmitter raw materials. Restoring these allows the brain to begin producing dopamine, serotonin, and endorphins again before treatment begins.
MEDICAL ELIGIBILITY
Contraindications & Screening
The safety and efficacy of ibogaine treatment requires that certain conditions and medications be absent or fully resolved before treatment. These are not optional guidelines — they are clinical requirements. If you or a participant has any of the following, a thorough medical assessment is required before proceeding.
Absolute Contraindications
- Prolonged QTc interval (>450ms on EKG)
- Pre-existing cardiac disease, arrhythmia, or structural heart condition
- Severe liver damage or cirrhosis
- Active epilepsy or seizure disorder
- Schizophrenia or active psychotic disorder
- Bipolar disorder (Type I — active mania)
- Current use of MAOIs, antipsychotics, or TCAs
- Current use of methadone (until fully transitioned)
- Untreated thyroid disorder
- Active alcohol or benzodiazepine dependence (medical detox required first)
- Pregnancy
- Severe uncorrected electrolyte abnormalities
- Use of QT-prolonging medications within washout period
Medications Requiring Washout / Tapering
- SSRIs — 2–6 weeks (fluoxetine needs 5–6 weeks)
- SNRIs — 2–4 weeks (slow taper essential)
- Buprenorphine/Suboxone — 3–6 week taper + bridge opioid protocol
- Benzodiazepines — Must be stabilized; do NOT abruptly stop
- Bupropion — 2–4 week taper
- QT-prolonging antibiotics — 7–14 days after completion
- Azole antifungals — 7–14 day washout
- 5-HTP / L-Tryptophan — Stop 2 weeks prior
- St. John's Wort — Stop 4 weeks prior

Having spent many years training in the Amazon and apprenticing within indigenous plant medicine traditions, Max brings a rare combination of ceremonial wisdom and therapeutic precision to his work. His years of dedicated experience working specifically with Iboga and Ibogaine have shaped the core philosophy of Entheos — one that honours the full depth of what these medicines can offer when held with integrity, skill, and genuine human care.
